Renovation of Philippines Medical System/The impact of medical system on the country’s health and well-being

The Philippines, a third world country, is getting a tremendous amount of support from their allies and Non-Governmental Organizations. Nonetheless; the death rate increases caused by curable diseases such as Ischaemic heart diseases, Neoplasm, and cerebrovascular diseases. The root cause of the constant rising death rate is due to the unequally distributed number of doctors in the country. Therefore, this study aims to address the medical personnel conflicts for the enhancement of public health and medical service and as well as the well-being of the nation through suggestions. Researcher maintains that the unequal allocation of medical personnels is resulting from salary disparity and unequal support from the government. Data was gathered using a survey of 15 students conducted from a private school of the philippines located in Biñan. Majority of the respondents desired to work in NCR and CAR due to high guaranteed salary. Therefore, salary disparity  is highly associated with inadequate medical personnels in a certain region. The researcher emphasizes the renovation and implication of governmental doctor policy in order to attract qualified candidates for optimization and provide equal manpower in each region.

    I just have a quick question about your presentation.
    If governmental doctors has a similar salary with private hospital doctors, private hospitals might lose their motivation to make their private hospitals and they will try to join governmental doctor segment. In this situation, how can the government sufficiently collect funds to pay these doctors? Also, how can the government guarantee the medical quality of the government doctors compared to private hospitals? could you share your idea about this question? thank you.

      Good day, I am Song Si Hyeon, a participant from De La Salle University. These are the responses as regards to the questions raised.

      The government can set a restriction or limit on maximum capacity of candidate selection biennially. In this way, as the acceptance rate of the governmental hospital recruitment is low, the government will be able to select only the proficient and consummate candidates. Thereby, the government can guarantee the medical quality of the government doctors. Furthermore, there wouldn’t be a problem regarding unequal distribution as well since there is a settled maximum capacity of doctors that the governmental hospital will govern or handle. In my opinion, the concerns in terms of the budget or funding has to do with the corruption problem. The Philippines is greatly suffering from the impact of widespread corruption which severely restricts the efficiency of the country’s economic development/growth. I believe that just by addressing the issue in relation to the government, it would be possible for them to be possessed with sufficient resources and funds which are to be distributed to the governmental hospitals.

I believe that the policy proposed in your presentation well-reveals the problem in the Philippines regarding irregular distribution of doctors throughout the country, and furthermore, I also got to learn about the severity of this issue. So thank you very much! I have some questions I would still like to ask you, though. Firstly, did this disparity in different aspects worsen during the pandemic, and secondly do you believe that your policy proposal can be initiated in the near future?

      Apparently, Republic of the Philippines Department of Labor and Employment have declared that all private sector medical personnel in the National Capital Region are receiving higher daily wage rates compared to before the pandemic, due to their high exposure to the health risks and for an Emergency Cost of Living Allowance (ECOLA). On the other hand, local hospital unions are going abroad or requesting for incentives/compensations to seek for their respective family needs. This will in fact worsen the situation of the brain drain in the Philippines and will make a wider gap between the local and national healthcare workers. I believe that the proposal I have presented can be initiated in the near future because recently, Filipino healthcare workers are protesting and calling for the government to lift a deployment ban to prevent the medical staff from leaving the country to work abroad. As a result, the government would prepare for implementation regarding this situation, and I am optimistic that my proposed policy can be initiated soon.

    그룹 6에서 아동 포르노 학대를 연구한 개인 참가자 박수빈이라고 합니다. 필리핀 의료 시스템의 혁신에 대한 시현 님의 연구는 상당히 흥미로웠고, 그간 국제 분야에서 보기 드문 주제 중 하나 였습니다. 급여 격차와 의료인의 불평등 한 배분에 대한 조사에 있어 새로운 정보 소개와 더불어, 다소 관과되기 쉬운 저속득층 국가의 의료 부문에 관하여 다양한 지식을 접할 수 있어 좋았습니다. 정부 의사 정책에 대한 시현 님의 제안 역시 상당히 신선했으며, 마찬가지로 높은 현실 가능성이 수반하는 전략이라 생각합니다. 개인적으로 해당 전략이 필리핀 시민들을 상대로 적용되는 상황 즉 관련된 구체적인 사례 / 전례에 접근하고 싶습니다. 해당 정책의 예상 기대 효과에 대해서도 부연 설명 부탁드립니다.

    Thank you for your great presentation! I think you analyzed the serious situation in the Philippines and the problems of the medical system from an economic perspective and told me well how the government’s policies should be implemented. However, because there are multiple factors involved in every problem, I thought it would be better to explain the factors in a more subspecialized way and provide more diverse solutions. Can you suggest other factors except salary disparity and unequal support? And what can be private-level efforts? Thank you!

    Hello! I am Hyoungbin Koh from the SDG 5. Your video and presentation were very interesting to watch regarding the medical system in the Phillippines. However, I have one question to ask. For the data, you have collected (the pie charts in the research paper), are the results being generalized to the entire population of the Phillippines? Isn’t the sample size too small for that data to be considered reliable for the entire medical system in the country? Or was this just a survey designated to show some statistical support? If you could clarify this point, I think that the research would be much more convenient to understand. Aside from this, I found the research to be entertaining and informative. Thank you!

      As I have mentioned multiple statistical data in the video presentation and the paper, this survey was conducted to support the other existing statistical data. The number of doctors in the Philippines by region in 2020 were recorded and the data that I have gathered was to explain that even the students aspiring to become a doctor in the future share similar thoughts with the current doctors. In this way, the survey became the supporting evidence that the salary disparity is one of the major factors affecting the unequal distribution of healthcare workers in national and local hospitals.

    Hello Si Hyeon! This is Seoyun from TAT. Thanks for the wonderful and remarkable presentation and the report that you have presented. Your idea of regulating the doctor’s income disparity is pretty impressive! Nevertheless, I have couple of questions below.
    1. Do you think Philippine’s overall medical system would be improved than these days through the implementation of the policy?
    2. Will there be any other possible unprecedented problems that would occur once the implementation is being enforced?

      Good day! Thank you for your feed backs. Here’s an answer to your questions.

      Firstly, although the implementation given would not be able to completely eradicate all the medical system issues in the Philippines, I am optimistic that the implementation of the given policy would definitely improve the medical system of the Philippines. Since one of the major problems of the Philippines medical system is brain drain, the Philippines does not only have problems with unequal distribution of doctors between national and local hospitals, but also they are experiencing scarcity of medical personnel. In order to improve the overall medical system, I believe that the country must use the implementation of the given policy regarding the compensation/salary in order to not waste several skilled candidates since the reason why Philippines doctors working abroad is due to insufficient salaries.

      For your second question, apparently, there would be unprecedented problems that may occur due to the enforcement of the implementation. For instance, a lot of patients would prefer government hospitals over any other hospitals since they are believed to be more reliable and cheaper since tax money is used for providing many public health services for the benefit of all citizens. However, there always are following consequences to any decisions or amendments. In this case, I would like to suggest that the government have an alternative solution to help the private hospitals to attract sufficient patients or to provide some medical equipment or support such as hospital beds, PPE, or aid kits to private hospitals for them to lessen the healthcare cost as an example.

I would like to ask, about your proposed solution, since it’s such a great idea to solve the health problem, what is stopping the government from implementing it? And also, if your proposed solution were to be implemented, what could possibly be the limitations? Thank you.

      Factor that stops the government from implementing is due its violation of doctors right on choosing a place of work. Just as other job workers, everyone has the right to choose their workplace. In addition, if the doctors would be working in the area where they do not desire to work, they would have several complaints regarding the working conditions. Job satisfaction is an important factor which motivates the workers to effectively work and to have remarkable outcomes. Consequently, job dissatisfaction would lead the doctors to decrease in productivity and might cause a severe problem of the citizens’ health at large. The possible limitations of the proposed solution if it was to be implemented is that the private hospital would not be benefited much from this implementation. For this reason, the doctors would desire to work in government hospitals since they get to be privileged.

    I saw both the presentation video and the report, and it was very interesting to watch.
    I have few things to ask in the contents of the presentation.
    By placing physicians appropriately and evenly, we would like to have children work in areas other than NCR and CAR where income is low.
    Is there anything we can do to ensure proper and equal placement of physicians?
    Also, is there anything I can do to set up public hospitals in each region of the Philippines?
